Job Description Certification Details - BLS - Certification by
the American Board of Cardiovascular Perfusion - State of Louisiana
professional license Job Details - Perfusionist position involving
call duties. Job Requirements - Certification by the American Board
of Cardiovascular Perfusion required. - Active professional license
for the State of Louisiana required. - BLS certification required
(AHA or ARC). Schedule Information - Call schedule. Additional
Details - Proof of COVID-19 vaccine or exemption accepted. - Proof
of MMR, Hepatitis B, Influenza, TB Test, Varicella, Tdap
immunizations or declinations accepted as per policy. - Fit Test
(N95 mask) required within 1 year prior to start, updated annually.
- Annual TB Questionnaire required within 6 months of start. -
Health statement signed within 1 year of start stating ability to
work without restrictions. - Proof of negative drug screen within
30 days prior to start. - Various background checks and
verifications required including criminal background, education,
employment, and driving record. - Completion of FMOLHS Skills
Checklist, Competency Exam, Core Competencies, and other required
education packets and forms.
