Job Summary: The Patient Compliance Specialist I (LPN) will indirectly help the patient manage their health by reviewing/managing electronic medical records (EMR) and working alongside the patient's provider. The Patient Compliance Specialist will manage EMR so that all health data systems meet the standards/regulations of government agencies and accrediting professional organizations. Essential Duties & Responsibilities: Responsible for maintaining AHG patients' compliance with Disease Management, Wellness/Preventative services, appointments/scheduling, and follow-upMaintains confidentiality of all patient health information dataCorresponds/coordinates with AHG clinic nursing staff and AHG providers regarding patient careResponsible for answering and directing high level of phone callsMakes high level of outgoing calls to patients needing follow up services scheduledRequests, processes, and maintains medical records of hospitals, non-AHG clinics, and other medical facilities consistent with medical, administrative, ethical, legal, and regulatory requirementsEnsures that all patient medical information and records is entered into the computer database Assists in the preparation of regularly scheduled reports when neededOffice skills such as printing, emailing, filing, faxing, scanning and organizing will be used daily.Responsible for maintaining the confidentiality of all patient health information data (i.e. EMR, vnet, etc.) in a prompt and highly accurate mannerPossesses working knowledge of AHG and Patient Compliance policies and proceduresNavigate and pull reports from all payor portalPull all Care Gap data from payor portals to call for follow-up appointmentsHandle Patient Portal question/needsCall on Care Gaps for all payorsHandle grievance calls and enter into moduleOther duties as assigned Marginal Duties: --Occasionally performs other duties and responsibilities as assignedKnowledge, Skills, and Abilities: Graduate of accredited Medical Assistant (MA) school or graduate of accredited Practical Nursing (LPN) school 2+ years of experience in the medical field preferred Ability to work with limited supervision and to make decisions based on established policies and procedures Exceptional interpersonal, verbal, and written communication skillsCustomer service-oriented attitude and behavior Intermediate/advanced computer and computer systems skills Experience operating inside electronic medical records Ability to effectively communicate with clerical and management personnel within the work site Supervisory Responsibilities: This position has no supervisory responsibilities Supervision Received: Works under general supervision of the Supervisor of Clinical Quality Management and occasional supervision of the Director of Clinical Services-- -- Working Conditions: Overtime will be occasionally required Travel may be required but is reimbursed accordingly to our travel policy.
