To improve our patients' quality of life and to facilitate optimal health outcomes through cost- effective healthcare, the Nurse Practitioner will manage and diagnose patients' psychiatric disorders and mental health conditions, while educating patients and family members about mental health and medical conditions, preventive health measures, medications, or treatment plans. Essential Duties & Responsibilities: --- Performs physical examinations assessing, diagnosing, treating, managing, evaluating, and monitoring psychiatric disorders and mental health conditions. --- Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes. --- Performs treatment using various forms of counseling and psychotherapy --- Distinguish between physiologically and psychologically based disorders and diagnose appropriately. --- Administers preventative health services within prescribed guidelines and under instructions of the collaborating physician --- Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs --- Prescribes and dispenses pharmaceuticals, evaluates outcomes, and dictates changes in management levels of therapeutics --- Write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements. --- Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes. --- Makes referrals to health professionals, specialists, other clinics, and community agencies --- Develop, implement, or evaluate programs such as outreach activities, community mental health programs, and crisis response activities. --- Assists in educating patients in disease management, address barriers to patient goals, navigating the health care system, and connect with community resources for assistance --- Participates in quality improvement, daily huddles, and other patient care programs established by clinic requirements (e.g. health coaching, motivational interviewing, screening for depression and substance abuse) --- Maintains accurate and confidential medical records on all patients in a systematic format approved by Affinity Health Group. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education referrals and consultations --- Ensures that patient records are kept confidential consistent with Affinity Health Group's policies and procedures and HIPAA standards. Monitors HIPAA compliance within the work environment Marginal Duties: --- Assists/instructs other health care employees in the management of patient care --- Assist in reviewing and updating Affinity Health Group policies and procedures --- May participate as a preceptor and role model for Nurse Practitioner students --- Collaborate with interdisciplinary team members, including psychiatrists, psychologists, or nursing staff, to develop, implement, or evaluate treatment plans --- Other duties as assigned Job Requirements:Knowledge, Skills, and Abilities: - --- Master of Science degree in Nursing and relevant Nurse Practitioner training/certification --- Current, unrestricted, and active Nurse Practitioner license in Louisiana --- Board Certification in Psychiatric-Mental Health --- Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance. --- Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders. - --- Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and deformities. This includes symptoms, treatment alternatives, drug properties and interactions, ad preventative health-care measures. --- Exceptional interpersonal, relationship-building, verbal, and written communication skills --- Customer service-oriented attitude/behavior --- Ability to work with limited supervision and to make decisions based on established policies and procedures Supervisory Responsibilities: --- None Supervision Received: - --- Works under general supervision of the attending Physician and Director of Clinical Services - - - - - Working Conditions: --- This position operates in one or more professional clinic environment with opportunity to work-from-home upon meeting eligibility requirements --- This is a full-time or part-time position --- Overtime may be required - --- Travel may be required at times Physical Requirements: - The physical requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
