Vantage is currently looking for a Medication Therapy Management Pharmacist to work at our downtown Monroe location from 8:00AM - 5:00PM, Monday through Friday. The Medication Therapy Management (MTM) Pharmacist in an integral member of the Vantage Health Risk Management Department dedicated to actively engaging members who are experiencing medication-related problems, are on complex medication regiments, or who are experiencing transition of care issues. The MTM Pharmacist works with those members and their prescribers to solve and document medication-related problems and recommendations to improve health outcomes and reduce cost. The MTM Pharmacist will perform a dual role in Medication Therapy Management and Medication Adherence. Essential Duties & Responsibilities: Serves as a pharmacy subject matter expert and drug information resource for members, clinical staff, providers, and Vantage staff for the assigned pharmacy function and/or medication therapy management program Identifies members through a variety of sources that are most actionable in terms of improving pharmaceutical care and positively influences them and their providers with the goal of improving health outcomes and reducing total cost of care Maintains a thorough working knowledge of pharmacy benefits (e.g. formulary, outreach programs, utilization management tools and techniques, medical policies) and works one-on-one with members and their providers to solve medication related problems Answers phone calls from members, doctor's offices, pharmacies, etc. Delivers MTM services to MTM eligible and non-eligible Medicare members, including but not limited to, comprehensive medication reviews (CMRs), disease specific medication reviews, star measure assessments, and targeted interventions to assess appropriateness of all medications, identify barriers to adherence, optimize medication therapy and improve clinical outcomes Generate deliverables to include Personal Medication List (PML) through comprehensive medication reconciliation, patient-specific Medication Action Plan (MAP) and written/verbal communication and feedback to healthcare providers Assist with managing medication costs through cost-effective interventions which may include, but are not limited to, formulary or therapeutic interchanges, combination products and assessing unnecessary and/or duplicate medication therapies Efficiently assess prescribed medication therapy for unnecessary medications, need for additional medications, member educational gaps, ineffective medications, inappropriate doses, adverse drug reactions, adherence concerns and opportunities for medication synchronization Communicate professionally and discuss/document clinical recommendations/interventions to prescribers and interdisciplinary care teams Follow-up with members and/or caregivers with an additional follow-up phone call subsequent to the initial patient interview when necessary and appropriately document outcomes in a timely manner Ensure members, family members, and caregivers have a positive experience by providing a high level of customer service by always responding promptly, professionally, and politely Meet productivity and performance expectations set forth by supervisor Data entry of patient interviews and medication reconciliation into web based MTM software. Documentation of communication with members, pharmacies, and providers in web based MTM software, Vnet, Microsoft Excel, etc. Understand CMS/HEDIS Star Measures, assess whether a member has met the measures they fall into, and counsel the member on the benefits of meeting these measures if they are not met Accuracy and Quality: training and continued education for HIPAA, The Joint Commission, OSHA and Vantage's pharmacy practices as relevant Stays abreast of CMS current guidelines for MTM and ensuring compliance with said guidelines Manages a high volume of e-mail communications Maintains focus to achieve individual and team productivity goals while maintaining a high level of accuracy Setting up and maintains member records as a result of medication therapy management outreach Serves as a Preceptor for rotating pharmacy students Assists with Medication Adherence as needed Job Requirements:Knowledge, Skills, and Abilities: Pharm.D. or B.S in pharmacy required Current Louisiana pharmacy license in good standing is required At least (3) years of work experience as a licensed retail pharmacist preferred Proficient in windows-based programs Knowledge and understanding of the Core Elements of MTM services Medication Therapy Comprehensive and Targeted Reviews Personal Medication List (PML) and Medication Action Plan (MAP) Interventions and Referrals Documentation of Services and Follow-up Excellent listening skills - present a warm, friendly, and empathetic demeanor demonstrating active listening techniques Clear and concise communication skills - able to articulate information, both verbally and in writing, in a clear and concise professional and grammatically correct manner Excellent interpersonal skills - demonstrate empathy with members, family members, and caregivers in tone, language, questions, etc. and demonstrate patience, caring, sensitivity, empathy, and cultural competency throughout job responsibilities Exceptional interviewing skills - able to probe and question members or health care providers to extract information to understand concerns and needs Conflict resolution skills - able to defuse potential member conflict utilizing the appropriate tone, support, questions, or counsel and respond in a supportive manner Maintain professional attitude and behavior at all times Strong work values - able to follow all company and departmental processes and procedures Strong quality orientation; detail oriented; ability to learn and adhere to compliance/audit requirements Strong organization and follow up skills; ability to effectively multi-task and maintain composure in a high-volume environment Strong analytical, decision making and problem-solving abilities Ability to be self-motivated and work independently as well as part of a team Solid business acumen and process orientation Ability to maintain confidentiality and appropriately handle sensitive information with tact and discretion as required by HIPAA and company guidelines; abide by state pharmacy laws Familiarity with Electronic Medical Records whether hospital or office-based Disease management experience, clinical training or disease state credentialing/certification are preferred Immunization administration certification is preferred Working Conditions: This position operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, and photocopiers. This position will require viewing computer screens and typing on keyboards for prolonged periods of time.
