Job Description:

The MDS Coordinator promotes the physical and emotional well-being of nursing home residents by using the Resident Assessment Instrument (RAI) and Minimum Data Set (MDS) to gather information and develop personalized care plans. This role ensures all care plans comply with medical and ethical standards.Key Responsibilities:- Oversee daily MDS office functions and ensure compliance with regulations.- Coordinate MDS services with other resident services for continuity of care.- Complete necessary administrative duties, including medical forms, reports, and charting.- Participate in quality assurance programs and government agency inspections.- Manage resident admissions, transfers, and discharges.- Attend Care Plan and Standard of Care meetings.- Train staff on MDS processes and documentation.- Monitor and ensure accuracy of CNA Care Plans and resident notes.- Conduct MDS assessments and risk assessments regularly.- Maintain up-to-date Medicare documentation and schedules.Working Conditions:- Work in well-lit and ventilated areas.- Perform duties involving sitting, standing, bending, and lifting.- Handle frequent interruptions and interact with residents, staff, and visitors.- Communicate with medical staff and other department supervisors.- Work beyond normal hours when necessary.- Handle exposure to infectious waste and diseases.Education Requirements:- Graduate of an approved RN program.- Experience as an MDS Coordinator preferred, but on-the-job training will be provided.Join Our Team:We are seeking a dedicated MDS Coordinator to join The Springs of El Dorado.
