(08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health
Experience or 1 full year Hospice experience License: Must have
license in hand Certifications: CPR, BLS Must-Have: Must have
recent specialty experience. Proficient in HomeCare HomeBase (HCHB)
and OASIS. Description: Provides, coordinates, and directs the
provision of home nursing care according to physician orders. Care
is based on agency policies and procedures through the competent
application of the nursing process. Responsible for supervising and
evaluating Home Health Aides and Licensed Practical Nurses/Licensed
Vocational Nurses. Home visits and periodic office meetings are
required. Must have reliable transportation, a current driver's
license, and agency-required liability insurance. Average patient
load is 5-8 per day. Each type of visit and activity holds a
different point value, with a total expected productivity of 30
points per week. Types of cases include Start of Care, Wound Care,
and Follow-up Visits. Must be willing to travel up to 75 miles from
home. Mileage reimbursement is provided at the IRS standard rate.
The contractor will be assigned various patients that may equate to
roughly 6-8 points. No guaranteed hours are provided. If unable to
meet financial needs due to low patient census, alternative
arrangements can be discussed. On-call responsibilities may vary by
location and profession. All documentation must be completed within
24 hours of the appointment with the patient. Delayed and
incomplete documentation that impacts billing and follow-up patient
care will affect future assignments. Candidate must accept all
patients on their schedule regardless of distance from previous
