Role of the Primary Care Physician (PCP) The PCP in our organization is expected to: Be accountable for patient outcomes: The PCP takes responsibility for delivering strong clinical care and ensuring cost-effectiveness for a panel of up to 450 patients. They recognize the importance of building trust to positively influence patient outcomes. Act as a health coach: Rather than merely addressing illness, the PCP assists patients in setting and achieving both short- and long-term health goals, regularly following up to support their journey toward better health. Simplify actions for better results: The PCP prioritizes tasks to make behavior changes more achievable for patients and guides their care teams toward performance goals. As a value-driven organization, we regularly assess various metrics to support PCPs in achieving success and partnership status, including patient admissions per thousand, appointment utilization, patient experience scores, clinical gap closures, and medical cost measures. Each PCP will have specific targets to work towards in collaboration with center and market leadership, as well as their care teams. Fostering a positive culture of love, accountability, and passion is essential, and PCPs are expected to lead this effort. The PCP must demonstrate the ability to work both independently and collaboratively with other healthcare professionals. They will closely coordinate with managers and medical directors to ensure adherence to guidelines and participate in risk and quality management initiatives, clinical meetings, and other relevant gatherings that promote patient health and organizational objectives. Compliance with departmental goals, performance standards, regulatory requirements, and quality patient care policies is mandatory. Key Responsibilities Provide independent care for patients with both acute and chronic conditions, particularly in older adults. Take full responsibility for patient care and outcomes, seeking specialist consultations when necessary while remaining engaged in the patient's overall care. Engage with hospitalists for patients admitted to the hospital, regardless of their affiliation. Conduct assessments, diagnoses, treatments, management, education, health promotion, care coordination, and documentation for patients with complex health needs. Lead a care team that includes a medical assistant, care facilitator, and care coordinator for patients who can visit the office. For patients unable to attend the office—such as those in hospitals, skilled nursing facilities, or homebound—the PCP will collaborate with transitional care teams and other available resources. Participate actively in the management of their center and provide coverage for absent providers as needed. Each PCP is also expected to assist in patient recruitment for the center and the organization. Supervise and collaborate with Advanced Practice Practitioners (APPs), maintaining accountability for their actions while employed. Perform additional duties as assigned by management. Knowledge, Skills & Abilities Essential Competencies Availability and Accessibility: PCPs should be approachable and available to patients, facilitating trust through timely responses to walk-ins and communications. Service Orientation: Provide care that reflects the same quality one would desire for family members or themselves. Evidence-Based Medicine: Stay informed about evidence-based practices while recognizing the impact of lifestyle and nutrition on health outcomes. Physician Leadership: Continuously develop leadership skills to benefit patients, teams, and the organization. Quality: Strive for the highest quality of care, working collaboratively with care teams to achieve optimal outcomes. Influence: Effectively and compassionately motivate patients and teams to achieve the best results. Self-Care: Prioritize personal well-being to ensure the best care for patients, encompassing physical, mental, social, and spiritual health. Proficient in Microsoft Office Suite and other relevant software. Willingness to travel locally and occasionally nationally for training and recruitment purposes. Fluency in English, both verbal and written; additional language skills may be required in some locations. Exercise independent judgment in decision-making. Education and Experience Requirements MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty is required. Must possess or be able to obtain a State Medical License for the relevant state(s). Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred; board eligibility is required. Maintain board certification through necessary continuing medical education (CME) and/or re-examination as required. Must hold a current DEA number for controlled substances (Schedule II-V). Basic Life Support (BLS) certification from an accredited organization is required within the first 90 days of employment.
