The NorthStar -Community Health Worker (CHW) will be the primary
care navigator for members with complex behavioral health needs.
The CHW works assertively to locate and engage members in the
community to develop individualized plans of care that focus on the
member's definition of success and identify and address social
determinants of health (SDOH) or other barriers impacting members
ability to achieve their stated goals. This position will
collaborate with an interdisciplinary care team (ICT) to provide
comprehensive care navigation, improve clinical and quality
outcomes, and reduce total cost of care. CHWs will approach their
member work with an understanding of how inequities drive health
disparities and use this information to advocate and promote health
equity.
This position is full - time (40 hours / week), Monday - Friday.
Employees are required to work any of our 8-hour shift schedules
during our normal business hours of 8:00 AM - 5:00 PM CST. It may
be necessary, given the business need, to work occasional overtime.
This role is located in the parishes of Louisiana - Union, Lincoln,
Jackson, Ouachita, Caldwell, Morehouse, West Carroll, East Carroll,
Richland, Franklin, Madison, and Tensas.
We offer weeks of paid training. The hours during training will be
discussed.
